Module 1: Why Age-Friendly Care Matters
Examines demographic and clinical drivers for age-friendly care, introduces the origins and goals of the Age-Friendly Health Systems initiative and outlines the value of adopting the 4Ms in primary care.

Module 2: Understanding the 4Ms Framework
Defines each of the 4Ms, explores clinical applications and real-world examples and highlights how the 4Ms work together to support holistic, older-adult care.

Module 3: Overcoming Barriers to Implementation
Identifies common physician- and system-level barriers to adoption, presents strategies for initiating age-friendly practices and encourages reflection on readiness to champion the 4Ms in one’s care setting.
