As a member, AAFP requires you to belong to your designated chapter as well as the national organization. The AAFP has a chapter for each of the 50 states, the District of Columbia, Uniformed Services, Guam, Puerto Rico and the Virgin Islands.

How chapters influence AAFP policy
Two members from each AAFP chapter serve as delegates to the AAFP Congress of Delegates, the organization’s policy-making body. AAFP members can submit resolutions requesting their chapter or national organization take action on issues they’d like to see addressed. Resolutions are considered first at the chapter level, where leadership determines whether they should be submitted to the Congress of Delegates.
