    Cognitive Care in Family Medicine Settings Quality Improvement Project

    The Cognitive Care in Family Medicine Settings Quality Improvement Project aims to give AAFP members and other primary care clinicians access to an important care pathway that can facilitate the early diagnosis of mild cognitive impairment and dementia, with the end goals of providing education and training for family medicine settings to integrate cognitive evaluation, diagnosis and management in their daily workflows

    The application deadline is June 15, 2026. Selected practices will be notified by June 30.
